WEEK/UNIT 3
Guest Lecturers:  Documentation Nightmares/Documentation Fraud
            Guest Lecturer Sallee is a clinical documentation specialist and will present a one hour Power-Point presentation on pitfalls of documentation. 
            Guest Lecturer Gonyers is a nurse who has had her license reprimanded and has been through a criminal prosecution for documentation fraud. She will present a thirty minute lecture on the ethical principles ignored on her journey from respected nurse to criminal.

There are stories and there are stories!  This unit we will explore documentation dilemmas.  Have any of you any stories you would like to share?

    I am very sure I have failed in my charting in the past. It seems we even know the risks and then take short cuts of leave out informaiton. I have an acquintance in a running group who is a nurse and then a lawyer. She knows our responsiblities and what we should document. A formidible enemy if a case arises.

  2. I think that we have all fallen short in documentation. The dilemma is that if we really documented every single thing we do, we'd have no time to care for our patients. I am currently sitting on the group that participates in doing Root Cause Analysis for incidents at our hospital. It is terrifying to see how lack of documentation makes is look like poor care has been given, even knowing the quality of the staff member who gave the care and the high standards they hold.

  3. Thank you both for sharing and I agree that all have fallen short in documentation. I have worked with a few nurses who fell short and entered into the realm of criminal fraud. Beyond legal aspects of documentation, at some, point I would like to research ethical aspects of documentation.
