The Health Insurance Portability and Accountability Act of 1996 (HIPAA) took effect in 2003.
This act focused on identifiable health information of the individual. This information is any protected health information (PHI) that is produced in or by covered entities. Examples of covered entities are “individual health providers, health provider organizations, health plans, and health information clearing houses that engage in electronic health care transactions” (Cushman, 2011).
